Fall Registration
K-12 Student fee payment options are available. Forms can be found below, picked up in-district, or received via school mail after July 23, 2021. Families may:

Register & Pay Online through the Parent Portal on or after July 12, 2021.

Mail completed forms and/or payment to: FCSD ACT Office, 403 South 20th St, Fairfield, IA 52556.

Return completed forms and payment in person to the ACT Office.

ACT Summer Office Hours: Mon-Thurs. 7:00 a.m. to 4:30 p.m. closed on Fridays.

Attend August 5th at the FCSD Fee Payment Day.

Returning students must be enrolled for 2021-22 and a parent must have a Parent Portal account to register and pay online. Families new to the Fairfield District who need to enroll students for 2021-22 must go to the ACT Office. See address above.

Book Rental fees will be assigned to all students at the beginning of the year.
Families may qualify for partial or full waiver of book rental based upon the determination of a completed Iowa Eligibility Application. New eligibility applications cannot be processed until August 1, 2021.
Applicable adjustments will be made to these assigned fees after an approved signed wavier has been received. Optional fees are not subject to fee waivers. Go to Food Service for more information and Iowa Eligibility Application.

Each student must have an updated health form. If paying fees online, please mail form to ACT or drop off at 403 South 20th Street.
If student has had any vaccines updated then we will need an updated immunization form. All students entering 7th & 12th grade MUST have their Meningitis Vaccine before starting school.
Students in grade 7-12 participating in athletics must submit physical form
Students participating in interscholastic athletics, cheerleading, and dance - Form Required
The Family Educational Rights and Privacy Act (FERPA), a Federal law, requires that Fairfield Community School District, with certain exceptions, obtain your written consent prior to the disclosure of personally identifiable information from your child's education records. However, Fairfield Community School District may disclose appropriately designated "directory information" without written consent, unless you have advised the District to the contrary in accordance with District procedures.
Complete the FERPA Student Permissions form if you wish to deny one or all of the permissions or make changes to student's current assigned permissions. Current permissions can be reviewed on your Parent Portal. Student's current assigned permissions will be shown on the Census information update form and can be viewed through Parent Portal Permissions.
